This year we are requesting itinerary/appointment information for the coming year be submitted electronically. The link below provides the FORM for entering the information.
PLEASE READ THE INFORMATION BELOW BEFORE COMPLETING THE FORM it is IMPORTANT that the form not be modified or re-scanned back into a PDF format etc. PLEASE try to follow the steps below carefully !!
The AGM will provide the password for opening the file. Be careful entering the data because once submitted, it can only be changed by fully resubmitting a second time.
If Adobe Acrobat is not installed on your system, you will have to search it on the internet and install it. It is FREE. Other PDF file readers will not always permit entering data into the forms.
Please follow the following steps:
- “Right-click” on the green link below for the form.
- Choose the option to either “SAVE LOCALLY” or
“SAVE LINK AS”
- This will save the form on your local machine.
- (Remember where it is saved on your computer!)
- Open your file explorer and navigate to the saved form.
- Right click on the file just saved and select “OPEN WITH” then select “ADOBE ACROBAT”.
- Complete the form, then SAVE it. Do NOT change the filename.
- When you SAVE the form, do NOT do a “save as” or the format could be changed. Simply SAVE the form with the same filename.
- Open your email program and compose an email to Webmaster@PAEasternStar.org
- ATTACH the completed PDF file to the email and send.
You should receive a confirmation email within 48 hours.
IF you place the words “Itinerary Entry” or “Appointment Entry” in the subject line, you will get an automatic reply
NOTE: DO NOT use the email application within Adobe Acrobat to send the form as it may not operate with your system.
